Surgical Sperm Retrieval at Shivdikar Human Reproduction & Fertility Centre

When testes are producing sperm but there is no sperm in the ejaculate (azoospermia) surgical retrieval is necessary prior to IVF. Occasionally this may also be required when the husband/male partner is unable to ejaculate.
The procedures described below are usually carried out the day before or the day of egg collection. If there is concern that sperm might not be found, SSC may be performed before the woman starts her treatment. The procedure takes 15 to 20 minutes and may be done either under general anesthesia or under local anesthesia.
Sperms may be obtained either from epididymis (PESA) or from testes (TESE).
 
Percutaneous epididymal sperm aspiration (PESA)

This technique is suitable for men with blocked vas difference or those born with agenesis of vas difference that transmit the sperms from the testes. A fine needle is inserted into the epididymis and sperm are obtained by gentle suction. If this is unsuccessful, it may be necessary to perform TESE.
 
Testicular sperm extraction (TESE)

A biopsy needle is inserted into the testis. This takes a fine strand or core of tissue from which sperm can be retrieved. Several samples may be collected. The tissue from the testes is obtained by Andrologist (Uro surgeon) the one is expert in this technique. And these tissues are then minced and processed in the embryology laboratory to identify sperms using an Inverted microscope for use in the ICSI cycle with swim out technique. If sperms are present and excess tissue is available then it may be possible to cryopreserve for later use.
Both PESA and TESE procedures can be repeated in future IVF/ICSI cycles if necessary. In TESE procedures multiple sites are used to retrieve the sperms.
